- Health information (including medical conditions, allergies, medical requirements and medical history)
- Information about care needs (including disabilities, medication and dietary requirements and general care provisions)
- Records of meetings and decisions

- We're collecting the information because it is in your best interest that we are aware of past medical history. Treatments are not suitable for people who have certain conditions, so making sure we are aware of all past medical history reduces the potential risk of harm, and enables a personalised treatment plan to be made.
- Vital interests – collecting or using the information is needed when someone’s physical or mental health or wellbeing is at urgent or serious risk. All of your data protection rights may apply, except the right to object and the right to portability.

- We are collecting this information as it is a legal requirement as it is crucial for informed consent, proper treatment planning, and demonstrating adherence to industry standards of care and safety. It also benefits both parties to have a record of the consent process and in depth records of the procedures performed, in case any problems arise. It gives us the ability to look back and give an accurate account in a court of law should it be required.

- We are collecting and archiving this information as it is in both parties benefits to be able to look back and see what treatments have been performed, what area they were performed, what product was used, and how much product was used in each area, so when you come for further treatment we can look back and see the above in order to create a new suitable treatment plan. It is useful to retain the medical history information provided so that we can note any changes and any potential problems in subsequent treatment plans. It also benefits you to be able to access the information in order to pass on to other health care professionals i.e. your GP should this be required.
- Vital interests – collecting or using the information is needed when someone’s physical or mental health or wellbeing is at urgent or serious risk. This includes an urgent need for life sustaining food, water, clothing or shelter. All of your data protection rights may apply, except the right to object and the right to portability.

- We are collecting this information as it benefits both parties to have a record of what treatments have been performed, what area they were performed, what product was used, and how much product was used in each area in case a problem arises/complaint is made/or you have a query about the treatment. It allows us to look back and be able to address any queries, complaints or claims more accurately.

How long we keep information
Records will be kept for a minimum of 8 years after the last treatment. At the end of the minimum retention period, a review will be conducted to determine if the records should be archived, destroyed, or kept for further retention.
Who we share information with
Information will only be shared if there is a need to do so, for example, in emergency situations, or if a problem arises that needs further medical attention, or for any legal proceedings. In the event that information needs to be shared it will only be shared with those directly involved in further care and/or proceedings. We will always aim to do this with your consent where possible.

Photos and/or videos of treatments may be shared on social media and websites with your prior consent, however no personal information will be attached to these.
Duty of confidentiality
We are subject to a common law duty of confidentiality. However, there are circumstances where we will share relevant health and care information. These are where:
- you’ve provided us with your consent (we have taken it as implied to provide you with care, or you have given it explicitly for other uses);
- we have a legal requirement (including court orders) to collect, share or use the data;
- on a case-by-case basis, the public interest to collect, share and use the data overrides the public interest served by protecting the duty of confidentiality (for example sharing information with the police to support the detection or prevention of serious crime);
- If in England or Wales – the requirements of The Health Service (Control of Patient Information) Regulations 2002 are satisfied; or
- If in Scotland – we have the authority to share provided by the Chief Medical Officer for Scotland, the Chief Executive of NHS Scotland, the Public Benefit and Privacy Panel for Health and Social Care or other similar governance and scrutiny process.
